The transition from being a detained inpatient to living in the community can be a daunting, difficult and risky time for patients. When patients have had relatively long periods of admission or are moving on from secure services, this transition requires careful management.
The Community Forensic Transition Team are a multi-disciplinary team who will offer a package that comprises two distinct but overlapping and interlinked components to develop, support and expedite transition plans for patients who have been identified for discharge from forensic learning disability inpatient services at Northgate Hospital.
How to get referred
Referrals come directly from MDT within medium, low and locked rehab services, or direct from the individual’s responsible clinician. The referral is then discussed at fortnightly team meetings whereby the patient referral is accepted and commenced onto our caseload.
