The South Northumberland Adult Community Treatment Team, based at West Farm House in Cramlington, provides assessment and treatment by a multi disciplinary team. Often when people are experiencing difficulty with their mental health they will need to be seen by a specialist service to assess their current mental health needs and see how they can be helped. Appointments with the team are arranged at a number of different sites.

Who is the South Northumberland Adult Community Treatment Team for?

The team provides a service for men and women over the age of 18 years who are experiencing difficulty with their mental health and require a period of assessment and treatment. A range of interventions and therapies will be offered to suit your personal needs. This can include group work, training and health checks.

How can I get referred?

All referrals to our working age (18-65 years) adult community treatment teams in Northumberland will be sent to our single point, where referral will be triaged.  A referral template has been developed for inclusion in patient information systems which you can access here.

All referrals using this template should be addressed to our single point of referral address [email protected]

Each referral will be processed via our multidisciplinary referral triage HUB, which is manned daily between 10am and 12 noon with rostered clinical and admin staff from across the county.  Working in conjunction with Consultant Psychiatrists, Pharmacists, Nursing Staff, OTs and administrative personnel, we will aim to provide the quickest possible response to all referral requests, identifying the most appropriate course of action, including assessment, advice, medication queries, signposting and onward referral.
